Jay Cook

Harlan "Jay" Cook age 62 of Russellville died Monday, January 28, 2019 at Select Medical Center in Fort Smith. He was born June 7, 1957 at Kokomo, Indiana to the late Ted and Lula Verser Cook. He loved sports and spending time with family. He was preceded in death by his parents and one sister, Vicky Cook.

Survivors include one daughter, Heather Perry (Jeff Reeves); one sister, Beth Bryant and husband Rob; three grandchildren, Dillon Perry, Sophia Perry, Baker Reeves and two nieces, Andrea Anders, Stephane Bryant.

Memorial services will be held at 5:00P.M., Thursday, January 31, 2019 at Cornwell Chapel in Dardanelle.
